Important: This page contains information about what you need to know for open enrollment and changes effective January 1, 2025.

8×8 is focused on continuing to provide a competitive health and welfare package to our employees. We encourage you to read through the below important information regarding changes and reminders as you evaluate your needs during open enrollment for coverage in 2025.

Open Enrollment is your chance to…

  • Plan changes taking effect January 1
  • Definition of eligible dependents
  • Current beneficiaries
  • Your 2024 medical, dental, vision and voluntary life elections without a qualified life event

If you wish to make any changes to your current elections, you must submit those changes during the open enrollment timeframe for a January 1, 2025 effective date

2025 Flexible Spending Account(s)

  • Healthcare, Dependent Care, and Limited Purpose FSA must be elected every year
    • 2024 elections do not rollover to the following year
  • Commuter FSA elections do rollover, you can adjust contributions at any time directly at HealthEquity

2025 Health Savings Account (HSA)

  • Required in order to receive the 2025 Employer contribution
  • Elect a $0 or up to the maximum amount as an employee contribution
  • Employee contributions can be changed at any time

Eligible dependents

  • Spouse, domestic partner
  • Children up to age 26
  • Disabled children only – subject to carrier’s disability process

Important Reminders

  •  Open enrollment benefit selections will take effect on January 1, 2025.
  • You can use Benefitsolver’s Decision Support tool for help selecting the benefits that are a good match for you.
  • Take Advantage of Sofia, your personal benefits assistance, during enrollment for answers to the most asked questions.

What if I take no action?

  • 2024 elections for medical, dental, vision and voluntary life plans will rollover to 2025
  • 2024 FSA elections will default to $0 for 2025.
  • 2024 HSA employer contribution will default to $0 as of January 1, 2025.

Medical

Personify Health (Formerly Health Comp)/Anthem

Our medical network is not changing but HealthComp is re-branding to the name Personify Health. You will start to see this name throughout our communication materials.

We will continue to offer a PPO and PPO with HSA plan option in 2025 for our nationwide plan options. These plans have minimal changes for the upcoming year, as outlined below:

PPO Plan

  • In and out-of-network deductible increase to $1,000 individual/$2,500 family
  • In-network out of pocket maximum increase to $4,000 individual/$8,000 family
  • Out-of-network out of pocket maximum increase to $4,500 individual/$9,000 family

PPO w/ HSA Plan

  • In and out-of-network deductible increase to $2,000 individual/$4,000 family
  • In-network out of pocket maximum increase to $4,000 individual/$8,000 family
  • Out-of-network out of pocket maximum increase to $6,500 individual/$13,000 family

Pharmacy

National CooperativeRx and CVS Caremark

We are excited to announce, beginning on January 1, 2025, CVS Caremark will replace Liviniti as our pharmacy partner. With this transition to CVS Caremark, the pharmacy benefit will remain similar to the prior year, ensuring you have continued access to medication coverage and a national network of pharmacies. However, there may be some differences in which medications are covered and how they are covered under the pharmacy benefit. To confirm if your current prescription is covered by CVS Caremark, visit caremark.com beginning in late December, 2024 and select the Advanced Control Specialty Formulary. CVS Caremark will be sending a letter to members impacted by a tier or coverage change.

Resources During the Transition to CVS Caremark:

  • If you have any questions related to the transition to CVS Caremark, you may call the Customer Care Team at 1-866-818-6911 beginning on January 1, 2025.
  • Visit the National CooperativeRx and CVS Caremark benefits page for more information, including how to find an in-network pharmacy, register for the member portal or mobile app and more.

What does this mean?

Before January 1st, you should try to get one last fill of medications on the old plan.

  • Liviniti will transfer any active prescriptions to the mail pharmacy vendor with CVS Caremark so you won’t have to obtain new prescriptions for mail. View the Caremark Maintenance Choice Flyer to get started.
  • Liviniti will transfer active prior authorizations to CVS Caremark for medications on the Advanced Control Specialty Formulary (drug list).
  • New Member ID Card: Due to the new pharmacy partner, you will receive a new ID card.

CVS Caremark Digital Tools

  • Find a pharmacy
  • View your claims history
  • Download a digital pharmacy card
  • View which drugs are covered under your plan

Your Prescription Coverage Reminder

Ensure your prescription is processed through CVS Caremark and not Anthem. Anthem is your provider network not your prescription coverage.

8×8 continues to focus on providing competitive options to our population in effort to provide choice to suit you and your family’s varying needs. Contribution changes for the 2025 plan year can be found under the contacts and resources section of our 2025 open enrollment webpage. A high level summary by line of coverage as follows:

Personify Health (Formerly Health Comp): Contribution increases on a per pay basis for all plan options and tiers

Kaiser: Contribution increases on a per pay basis for all plan options and tiers

Dental: Minimal contribution increases on a per pay basis for all plan options and tiers

Medical Plan Updates

We will continue to offer two plan options in 2025. In effort to manage cost of the program effectively, there are plan design changes that will be taking place for both of these plan offerings in 2025. We encourage you to review the new plan design options outlined under the Kaiser section of our benefits webpage for 2025. Please note, due to these changes, you will see new Kaiser plan names incorporated into our communication materials.

New Employer Sponsored Health Benefits

Health Advocate

All Personify Health (Formerly HealthComp) medical enrolled employees will have access to Health Advocate, a resource to help you manage your healthcare or insurance issues. An experienced expert will take your issues out of your hands and resolve them on your behalf. This includes helping you understand your coverage, resolve claims and billing issues, understand health conditions and more! Noom

Noom is a behavioral change program that offers personalized coaching, educational content, peer support and tracking tools to help you achieve the health outcomes that you are looking for, whether that’s losing weight, managing diabetes, support while taking a GLP-1 or improving your overall health. This program is available at no additional cost to all nationwide PPO plan enrollees and covered dependents. Dependents under the age of 18 are excluded.

One Medical

One Medical, a membership-based primary care doctor’s office, will be available to all 8×8 employees and their covered dependents enrolled in a Personify Health (Formerly HealthComp) medical plan. One Medical helps with:

  • Everyday care and prevention
  • Care for urgent concerns
  • Colds, flu and COVID-19
  • Mental health
  • Sexual health
  • Chronic conditions
  • LGBTQIA+ services

Book same/next-day appointments, request prescription renewals, and schedule remote visits with a provider if you can’t make it into the office — all from their app. And just like a regular doctor’s office, One Medical works with your insurance. Enhanced Voluntary Benefit

PetPartners

8×8 is pleased to introduce pet insurance, now available through PetPartners, as a new benefit to help protect you and your furry family members from unexpected vet expenses for covered accidents or illnesses. Dogs and cats can be enrolled from 8 weeks to 13 years old and once covered, the policy will remain active regardless of age. To enroll in coverage, visit your benefits enrollment portal, BenefitSolver.
